Please read these directions completely before continuing to complete this form online. Your request for renewal cannot be processed if it is incomplete, illegible, or includes false statements. 

You are eligible for the Nurse Aide Competency Exam if: 
  • You have completed an approved training program.
  • You are eligible to challenge.  
INSTRUCTIONS: 
  1. Complete and Submit this Nurse Aide Testing Application.
  2. Once approved, you will receive an email with your Username and Temporary Password to sign in to your TMU© account.
  3. Please refer to the D&SDT-Headmaster's Iowa Nurse Aide web page and the Iowa Candidate Handbook, found at www.hdmaster.com, for instructions on 'Completing Your Account' and other important testing policies and updates.
Once you have completed all the fields in this application, select 'Send Application' to submit it.
